Initial Appointments
You must have a referral from a GP, specialist or nurse practitioner to attend this practice.
Your appointment will be made once we have received your written referral.
All referrals are checked by our Specialists to ensure urgent cases are seen promptly. Our staff will then contact you (allow up to 3 weeks) and offer an appointment that is suitable to you.
NOTE: Please phone rooms directly for appointments with Imogen Batten (Physiotherapist), Tracey Stewart (Midwife), and for pelvic floor chair treatments.

Before your appointment
Please complete the Patient Registration & Privacy Form online by clicking the button below or on arrival at the clinic. If completing at the clinic, please arrive 15 minutes before your appointment time.
What else should you bring to your appointment?
Relevant health cards:
- Medicare card
- Veterans Affairs card
- Private Health card
- Pension Card
The referral from your doctor or specialist (required at the initial consultation unless the referral has already been sent to our rooms).
Any relevant ultrasound films or reports.
Antenatal patients will need to bring their Antenatal Share Care Card if they have one.
Appointment Cancellation
We require a minimum of 24 hours notice for appointment cancellation. Patients who fail to give appropriate notice will incur a non-attendance fee. Our policy is that no further appointments will be scheduled until this fee has been paid in full.
Payment
Patients are required to pay for their consultation in full on the day of their appointment. Cash, EFTPOS and major credit cards are accepted. We do not Bulk Bill. We can lodge your Medicare claim electronically.
